He uses the real-life story of a young doctor's descent into morphine addiction as a parallel to the growing chaos as the Bolsheviks take power. The film is based on the writings of the controversial Russian writer Mikhail Bulgakov and you realize it is a veiled criticism of the Leninist revolution. It is dark with persecution of Jews and landowners highlighted.
