The ending involving the woman he previously trepanated 1 year ago was a bit confusing for me. For me, it's just Nakoshi's hallucination since he killed her 1 year ago, and he hallucinates about her because he firmly believes that every trepanation attempt is a success since he himself has survived so many attempts.

I think it's better to treat Homunculus as an absurdist work. There are definitely themes, and lessons, and psychological horror, but in the end it's just an absurd story.
So, I finished Homunculus earlier, and I was certain that at the end, Nakoshi kills Ito. I figured he lobotomized Nanako, and probably ultimately killed her. Then spent a year continuing to go insane and drill more holes in his head, and possibly kill/assault more people trying to get them to see homunculi.
