I imagine that the 'agreement' outlines certain conditions to which you are prepared to commit. While your signature at the end is in fact a proof of sorts that it is you that is entering the 'agreement', it is not necessary to state the fact.

I have seen proof-reading but I don't think that is accepted as standard, at least in the US. The hyphenated version proof-reading seems to be used in the UK at times, and in some other countries such as Singapore.
